The Oral Health Professional Educators’ Awards form

You are about to submit an application for the Oral Health Professional Educators’ “Practice Green” or the “Social Excellence” Awards. In submitting an application to one of the awards, you confirm acceptance of the following terms and conditions:

1. Your institution is an ADEE institutional or affiliate member with current years membership fee paid;
2. You have the consent of the academic and administrative lead of the institution to submit to the awards on behalf of the institution;
3. You authorise that the video submission you submit if shortlisted may be placed in the public domain via ADEE or Henry Schein Inc in whole or in part edited form for promotional and educational purposes;
4. You confirm that all persons included in your video submission have consented to their inclusion and that you have made them aware of these terms and condition of participation;
5. You confirm that if shortlisted at least one person from your institution will attend in person the ADEE 2026 Plenary Session in Budapest on Friday 28^th August and that all attending will pay the appropriate attendance fee;
6. You confirm that your or your institution will be solely responsible for travel, accommodation and all other charges incurred if required to discharge point 5;
7. You confirm that it your responsibility to put in place appropriate travel insurance in place and you release the ADEE and Henry Schein Inc from any claims, demands, liabilities, costs, or expenses that may arise from your attendance;
8. You authorise ADEE and Henry Schein, Inc., its subsidiaries and affiliates (collectively the “Company”) as [naming rights partner] of [ADEE’s Oral Health Professional Educators’ “Practice Green” Awards and the “Social Excellence” Awards] (the “Event”) to use your name as well as any footage (incl. video, sound, spoken word, pictures, likeness, etc.) of your participation at the Event in any of the Company’s public communication, press releases, and/or promotional material (whether print or electronic media) and you hereby release the Company from any claims, demands, liabilities, costs, or expenses in this regard;
9. You confirm that you are not being granted any financial compensation, nor do you expect to receive any now or in the future from the ADEE and Henry Schein, Inc.
10. You confirm that the decision of the judging panel is final and binding and no appeals will be entertained;
11. You acknowledge ADEE’s terms and conditions associated with the ADEE annual meeting available here;
12. You acknowledge ADEE’s GDPR and Privacy policy available here.

Please have your letter of support from the Dean or designated head of school along with your pre recorded video available to upload before progressing.
